Not 100% accurate. Did all this, even have Johnny at 85% (even though % apparently doesn't matter) and still did not get the option for this ending. Sitting here with Johnny going on 8 minutes. Maybe it's just bugged. I had a bunch of bugged quests this playthrough. Even after 3 years, numerous patches and fixes this game is still buggy as hell.
